''My doctors are very conservative,'' Mr. Kean said, in a telephone interview from his hospital bed. ''They don't like to operate unless it is absolutely necessary, and they say they have achieved some success in the past with this treatment.

''We can't talk about time frames, however, and that's frustrating,'' he went on. ''If I had a broken leg or a virus we could discuss when I could expect to feel better and go home, but with this type of injury we just have to wait and see what happens.''

The injury to the disk atop the fifth lumbar vertebra shows up on a CAT scan and any progress will be recorded. The pain is not in the Governor's back, but in his left leg, a result of a pinched nerve, he said.

The Governor maintains a busy work schedule with the aid of a computer terminal that has been moved into a sitting room near his bedroom. Members of the Governor's Security Unit, led by State Police Lieut. Joseph Eaton and augmented by troopers from northern New Jersey stations, are on duty around the clock.

In addition, the State Police bring three deliveries of mail, memos and official documents to the Governor's bedside daily. ''The last delivery arrives after supper and I usually get around to it at about 10 P.M. or 11 P.M.,'' Mr. Kean said. ''The other day there was so much they brought it in a fruit crate. Then I knew I was in trouble.''
